Most people hate the healthcare insurance industry (see almost universal support for Luigi allegedly shooting that CEO), but there is a multi-billion dollar propaganda industry to convince people that any change would only make it worse.
Also of course those who make money off of this scam are quite happy with things as they are, which includes many if not most politicians. This year so far, according to opensecrets, there has been $130 million spent on ~~bribes~~ lobbying by the insurance industry. Add on to that that a majority of congress are millionaires, and are presumably invested in things like insurance companies.

The average American thinks that allowing people to die from preventable or treatable illness will keep their grocery bills down. It is a death cult.
Know a nurse that worked in a medicaid/Medicare clinic. Basically place where people either low income, kids covered or no insurance and would be complaining about how they would come in for anything just to get say Tylenol or for easily preventable things but couldn't understand why they would go to the only place it'd be covered so they could actually afford it. I like how it still includes token fees for daring to use your health insurance that costs more than I make in a year. If they don't get that $10 on primary care visits, they might go bankrupt. And don't you even think about seeing a dentist for a $100 cleaning. That would absolutely ruin them.

you'd have to be earning... the equivalent of 1.3 million USD annually before being liable $2.5k/month for National Insurance. And the healthcare is actually free.

No, we have three separate elements here:
Premium - what you pay monthly, no matter what. The cost of entry.
Deductible - is the amount you have to spend before the insurance "kicks in". For Instance, on this plan, there is $0 deductible, which is VERY rare. In this instance, you'd start paying the co-pays right away. Normally, the deductible is a large amount of money. On those plans, you pay the full negotiated cost of the service, which could be in the hundreds or thousands.
Out of pocket max - this is the total amount you have to pay before the insurance covers everything, co-pays included.
Does it have a separate one for pharmacy out of pocket? Or like the rare 0 deductible also 0 pharmacy
