My adhd clinic 'misplaced' my diagnosis several times, I had to email a copy of it to my doctor in the middle of my appointment at one point.
The irony wasn't lost on me. The doctor didn't appreciate my pointing it out in the subject line of the email though.
“This will be the fifth time I have provided you with your own records of me. And I have become exceedingly efficient at it.”
