You basically set up a model train diorama without the trains, then use a tape measurer (instead of squares like chess). Dice are rolled to see if attacks and actions are successful. This is how most tabletop wargames are played, though there are some that don't use dice (the godfather of moderrn wargames, Kriegsspiel) or measurements (such as Crossfire by Arty Conliffe).
A lot of hex and chit wargames can have pieces replaced by models and terrain, though it's not as common. AFAIK there are Advanced Squad Leader and Steve Jackson's Melee communities that do this.
