Janeway had nothing to do with it that time. They found it on Museum-Voyager while doing some cleaning up or whatever shenanigans. A lot of people got fused.

Watched the whole thing. It's quite touching I must say, well worth your time. Except that one Chernobyl sequence, was a bit too long for my taste, ymmv.
His conclusion at the end, well, gotta admit it made me tear up a bit.

A freak transporter accident created Tuvix by joining Tuvok and Neelix into this dude, a person with distinct personality but somehow memories of both. The Status Quo, I mean Cpt. Janeway required that Tuvok and Neelix return, so he had to go back. Into the Transporter.
