Also it seems to me there's more obesity in rural areas. People taking transit walk a bit more than people that drive a car everywhere I guess.

It's been on the market for about 9 years.
It was approved in the US about 9 years ago. It takes time for it to then become available. Regardless, it didn't become available for weight loss until 2021, and there were shortages of it so it was not widely available.
