My doctor said that before I can get my estrogen I need a support group but I have scarcely any real friends :(

We can be your online support group. But what proof do you need to give the doctor? Do they need to meet your friends/see photos of you with them? Or is just telling the doctor about them enough?

Do online communities count as support groups? I hope so, especially for trans people who are often shunned by their direct surroundings.

There might be a mutual aid network near where you live. Getting someone to pretend to be your friend might be possible (though I have little experience with such groups). Since it seems difficult to prove that someone isn't your friend, it might be possible to just get your acquaintances to vouch for you.
