I don’t get it tbh. What’s the double meaning?
In the Simpsons, where the screenshot is from, the character shown has a thick Arnold Schwarzenegger type accent. Reread it with that in mind.

That’s the jokeahhh that makes sense. I have watched a lot of Simpsons but I’m deaf so I don’t catch the accents unless they’re explicitly mentioned in subtitles

There's a few variations on this joke, most of them involving a person whose accent/first language involves pronouncing j as y. Swedish, for instance.
