I doubt it's deliberate - someone used AI to summarize the report, and failed to validate the results.

if someone deliberately puts text into ai, and deliberately does not check the results, passes it to their team to also perform checks and also deliberately ignores false information...it's somehow not a deliberate decision in your eyes?
