From what little I watched of the show, he's not evil, he's just an asshole about things. Classic anti-hero.
I have watched the entire show and i can confirm. He's shown multiple times to actually be completely unempathetic and self centered, even with his only friend. He makes racist, mysogynistic, homofobic, etc, comments just because he wants to piss people off. I don't remember him saying anything transfobic but that's just because he never encounters a trans person he can insult (except maybe one time where he discovers a girl is actually physiologically a man and is completely unempathetic to her learning this information, despite knowing full well how much she identifies with her beauty as a woman)
If you're wondering how the show is appealing, he's a very interesting character to observe. The audience is meant to empatize with everyone around him and sometimes with him. He has some redeemning qualities that people try to focus on and they tolerate him because he's a net good. In the end people leave the hospital happy and healthy. He's extremely pragmatic and calls other people out on their bullshit all the time. He often unearths social tensions people have which are hurting them. He will stand for his principles and against other evil people. He's constantly a pain, but he's not spiteful, does not hold any groudges. If someone fucks up he either throws them out on the spot or they are forgiven completely

First off, there's no such thing as a physiological man. "Man" is a gender identity, and those are stored in the brain.
Second, she had an intersex condition. Androgen Insensitivity Syndrome IIRC.

The show is very funny, and he's not evil, he's just a jerk. He's transphobic for the sake of being an asshole, but if you came to him with gender dysphoria he'd give you a script for HRT and a pamphlet on bottom surgery. He's insanely effective at his job, and wholly committed to curing his patients. He just hates talking to patients and prefers them when they're unconscious.

I haven't played Disco Elysium, but I think what's happening here is that Harry's loins are singing to him "you've gotta get that lady's number" while both Harry and the lady in question believe this lady is a cisgender man. This results in an extremely uncomfortable situation for everyone involved where Harry tries to put the moves on someone he does not believe he is attracted to.
