Let's say I, a middle aged cis man went to my doctor, wanting gender affirming care, and my doctor wanted to provide - what, if anything, would the treatment/care consist of? Just making sure my testosterone level was up to Hegseth-approved standards?

Does this mean that the older term term "sex change" is more appropriate?

Frankly I prefer that to the wishy washy language around gender.

Yeah, I think wishy washy "too careful" language is annoying to everyone involved in the conversation. Overly vague language instead of just using the correct syntax serves nobody.

I don't remember the context or the exact question, but I do remember saying at some point, on the topic of gender and sexuality, something along the lines of "I was born with a penis, I like it that way, and I enjoy inserting it into opposite genitalia". I found that to be the easiest way of clarification. To my defense, this was before I learned what "cisgendered" meant and I just wanted to make sure there was no ambiguity. This was in 2010 or thereabouts.
