Say you're I'm hospital for something pancreas related, and they need to use something to manage your blood sugar long enough to get your body working right again. If you say that you've poorly tolerated a drug, they'll deprioritze not only that drug, but ones similar to it, in favor of ones that you've tolerated or haven't had at all, whether or not the original drug is a better choice.
I'm all for navigating a complex system with created data when it's necessary, but you really, really only want to do it when there aren't other options. And, as much as it sucks, the drug they're on is often better for the reason they're on it instead of others. Lying to switch back would be detrimental to their overall health
