BUT WISHING YOUR PENIS WOULD “FALL OFF” IS
A SIGN THAT THE DISTRESS IS INTENSE, AND YOU
DESERVE SUPPORT RATHER THAN HAVING TO
HANDLE IT ALONE.
PLEASE DON’T INJURE OR REMOVE IT. IF YOU
MIGHT ACT ON THAT THOUGHT, HAVE A PLAN, OR
ALREADY HURT YOURSELF, MOVE AWAY FROM
ANYTHING YOU COULD USE TO INJURE YOURSELF
AND CONTACT EMERGENCY SERVICES NOW.

The AI is "overreacting", but it still made me think. I only said that boners and arousal annoy me sometimes so much that I wish it’d fell off. No plans, although fantasies about accidents might happen.

You should definitely still tell them about how uncomfortable you are in it. It's totally understandable to not want to share everything with your therapist, especially considering how it can land you in a worse place. On the other hand there is still a lot you can share. If the therapist doesn't even respond positively to that, then you need a new therapist.
