Edit: there are two realities going on with this comic.

Reality 1: A tire complains about being tired (hah you missed it). The doc makes another great pun. The sad tire goes back to its spot to watch the sunset.

Reality 2: A patient complains. Instead of listening the doc jokes about it. The patient hangs him/herself.
