There aren't a lot of options here.

Either a church/neighbors take turns taking care of her

Her family takes care of her

Or she moves somewhere where she can walk/cab/state assisted transport to where she needs to go.

The US has shit for elderly safety nets.

I would assume if she's that close to the wire she doesn't own her own house she could sell and is already taking advantage of whatever welfare is available to make sure she doesn't end up in a ditch.
