Ladders? Milk crates? More seriously, in some places there are actual gates in the wall, which also allow ranchers access to their land on the other side.

Yep, here’s one in south Texas that’s about 1/4 mile from the actual border (the Rio Grande). It seems to be generally left open. The photo is from the “outside”.

Colour of the utility pole crossing over, making it extremely obvious that both sides of the fence are the same country.
