Serious question: what is it? Is it a real production car, or a custom Isetta, or a photoshop, or what?

Looks like, but it is not one.
I love tiny cars. If I could buy an EV Isetta, I would. Although my absolute dream car would be an EV Nash Metropolitan convertible.
Two tone red and white, just like that.
Depending on where you live, you might be able to get something like an updated Isetta EV.
We would have one in a heartbeat if they ever come to the US.
